Knowing When There’s “Nothing More to Learn“

Using this pathway improves the efficiency of a productive investigation program by allocating resources to incidents with greater learning potential.

But how do we know when it is safe to discontinue an investigation without leaving any gaps that could lead to incidents of higher learning potential? Failure to act responsibly in opting out of a full investigation can, and often does, trigger mandatory investigations later when the consequence is unacceptable.

This session demonstrates the use of a screening program created to identify process safety incidents that should be fully investigated, even if they appear to offer nothing more to learn. The assessment tool borrows from the American Fuel and Petrochemical Manufacturers (AFPM) Safety Matrix to identify a growing risk of safety hazards among particular tasks. A risk calibration method is then used to justify voluntary investigations to address low-consequence incidents as a measure to prevent mandatory investigations that exceed designated tolerance thresholds.
