3. Most Common Incident Investigation Mistakes Webinar – Barb Carr

Barb’s podcast about incident investigation mistakes really created interest.

4. The Best 5-Why Examples Podcast

This podcast is also based on one of our most-read ARTICLES. The article had lots of comments.

5. Why Human Error is NOT a Root Cause Webinar – Alex Paradies

This podcast (which was very popular) was based on two articles that received a lot of attention. See the articles HERE and HERE.

6. Debunking Bad Root Cause Analysis Methods Podcast

Debunking a myth is always popular (as the TV show indicates). This podcast also created quite a bit of interest.

7. Bonus Video – Mark Paradies Being Interviewed by Kathy Ireland on Worldwide Business

This TV show was filmed in Burbank, California, on a set next door to where The Tonight Show with Johnny Carson was filmed. Kathy Ireland is a very professional interviewer, and the whole interview was a great experience.
