What does a bad day look like? Hard-working puppy

hard-working

Sometimes, we forget to do something so something else won’t happen, and when that happens, we usually pay a price . . . as in the case of this hard-working puppy. He saw all the tissue paper and decided to help by opening all the packages!

Well, it was left out for the hard-working puppy to play with, right? (Note: When you have a puppy, you have to think like a puppy to avoid walking into something like the picture.)

When we forget to do something so something else won’t happen, we are, in general, inviting trouble. That’s why our TapRooT® System and instructors teach their classes to resolve precursor incidents and how to resolve precursor incidents.

The president/CEO of System Improvements Inc., Mark Paradies, is also the hard-working co-creator of the TapRooT® System.

Mark advises us that precursor incidents are “Minor incidents that could have been a major accident if one or two more Safeguards would have failed.”

Additionally, he informs us more about precursors and precursor incidents and why they are what they are (and why precursors/precursor incidents are not near-misses):

“A precursor to:

  • a fatality
  • a major injury
  • an environmental disaster
  • the loss of a major customer (quality issue near-miss)
  • a major financial loss

“Do those precursors incidents sound more serious?

“Would this terminology get management’s attention?

  • a precursor to a fatality
  • a precursor to a major injury
  • a precursor to an environmental disaster
  • a precursor to the loss of a major customer
  • a precursor to a major financial loss

“A ‘precursor’ implies that something WILL happen in the future if we don’t do something. We can be very close to a disaster that will happen if we don’t make changes.”

Why do industry leaders choose TapRooT® Root Cause Analysis to train their teams to be the best problem-solvers?
