UK Investigation, Leonardo AW139 Incident

The AAIB (Air Accidents Investigation Branch) investigation into a serious incident involving a Leonardo AW139 is ongoing.

The Leonardo AW139 suffered a tail rotor duplex bearing failure which was found during post-flight maintenance at Norwich Airport on June 13, 2022.

The helicopter is described by USA Leonardo, “The AW139 National Security Helicopter® is in use in the United States and around the world in multiple roles: Law Enforcement, Rescue, Air Medical, Fire Fighting, Border Protection, and Military Response, Airlift, Gunnery. These varied ‘government operator’ missions are conducted safely, effectively and economically.”

The investigation of the Leonardo incident has identified the failure mechanism within the bearing.

Work continues to identify the causal and contributory factors which led to the failure.
