RIP Jim Whiting
We Will Miss An Experienced TapRooT® Instructor
Jim Whiting was our first instructor in Australia. He brought a wealth of experience to our courses and was one of the world’s experts in risk management. He established TapRooT® RCA as a standard in Australia. He taught TapRooT® Root Cause Analysis to over 1,000 people worldwide and special risk management courses for TapRooT® Summit attendees. We were proud to call him our friend.
Therefore, we are sad to let TapRooT® Users know that Jim passed away in April of 2026.
Rest in peace, Jim.
Jim taught me a lot over the last two decades and frequently sent me articles and research he thought would be helpful. I will miss him and his great sense of humor.
You just never know when you will speak to one of your mentors for the last time.
Jim was a colleague and close friend to us in Australia. He assisted me with tips and tricks for training so I could become a licenced instructor. His joviality and strong personality were a good guide, never afraid to ask questions or to challenge people and their thinking.

Jim was a wonderful man. Very intelligent and willing to pass on his knowledge. I will certainly miss conversing with him at future Summits. Rest in piece Jim you have left a lasting positive legacy.
