What is the 2025 Improving Investigations & RCA Track?

When you register for the 2025 Global TapRooT® Summit, you’ll be given the option to select a track. A track is a recommended selection of sessions to attend for that given topic.

If you’re interested in bettering your investigations, whether for routine auctions or major accidents, we highly advise that you sign up for this track.

Feel free to contact Marcus Miller, the Track Leader, with any questions.

What are the best sessions in the 2025 Improving Investigations & RCA Track?

Investigations & RCA Best Practices Roundtable

This session is an open discussion hosted by three of our most celebrated TapRooT® Users: Brain Tink, Joel Solomon, and Pat Fortune.

Don’t miss the opportunity to network and exchange best practices with fellow practitioners who are passionate about root cause analysis investigations. Ask questions, get advice, and improve your RCA program.

Learn from the best in the field. This session takes place on Thursday, October 2, 2025, at 10:10 AM.

Why Asking Why is Never the Right Question

In this session, Jonathan Kennedy will discuss the importance of asking how before asking why while collecting evidence.

It is imperative to only collective data-backed and observable truths during the investigation. Otherwise, we might be searching for a preconceived root cause and reach a flawed or incomplete conclusion.

Asking why too early during a root cause analysis can soil the entire process. If assumptions are made before all the facts are laid out, critical details could be missed before they’re properly evaluated.

Jonathan Kennedy’s session will help strengthen your investigations with more accurate, current-state data. It takes place Thursday, October 2, 2025, at 11:35 AM.

Improvements Don’t Happen by Accident

During this session, Rick Hulse and Marcus Miller will help align your company and improvement goals.

For some companies, conducting a root cause analysis to find the best corrective action is just half the battle. Persuading those at the top to actually implement your proposed changes can be a steep challenge, especially without the proper communication skillsets and techniques.

Through interactive exercises, Rick and Marcus prepare trainees to communicate success metrics and harbor support from executive leadership.

Their session takes place on Friday, October 3, 2025, at 10 AM.
