The Need for Speed (Don’t Be Here Today; Gone Tomorrow!)

Need for Speed

How Fast Do You Need To Improve?

Speed

If you knew there would be a major accident, quality problem. equipment failure, production snafu, … tomorrow, you would know that you need to improve TODAY.

The problem with major accidents is that people don’t know they are coming.

They should know. Precursor Incidents have given them clues. But often, management ignores these warnings because they didn’t cause a serious loss. It seems as if they are waiting for a major accident to start the improvement process.

Here Today – Gone Tomorrow

I remember what a VP said after the root causes of a major accident were presented to him. He said:

“If anyone would have told me that these staffing cuts were going
to cause a major accident, we would NOT have gone forward with them.”

Strangely enough, the Division Director of the unit that had the accident was gone the next week. HERE TODAY – GONE TOMORROW.

You may have only one chance to stop the next major accident. If you are a senior manager, they probably won’t let you have two or three or more major accidents to learn your lessons.

If you don’t want to be here today, gone tomorrow, you need an effective plan to stop major accidents BEFORE they happen. And you need that plan NOW! You have a need for speed!

Make Your Plan

How do you develop a plan, and what should it include?

First, you need to identify and find the root causes of precursor incidents. This is sometimes called “Mining the Diamond.” Find out more about it here…

The same theory can be applied to quality incidents, production snafus, equipment failures, … .

Once you identify the incidents worth investigating, you need an advanced root cause analysis tool – TapRooT® Root Cause Analysis.

Why TapRooT® RCA? Perhaps this interview by Kathy Ireland will explain why you should be using TapRooT® Root Cause Analysis…
