TapRooT® Investigations and TapRooT® Process Improvements

Two speakers are scheduled to present during this session:

Mark Devlin

Mark Devlin, an HSE Professional from Australia with 15 years experience, will be sharing Success ImplementingTapRooT® Across an Engineering Firm the successful roll-out and ongoing support and maintenance of root cause analysis competency across a new, rapidly growing, multi-site, multi-national organization. This new Scottish based engineering, procurement and constriction company, (that split off in 2006 from a much larger US based global engineering, construction and services company), was keen to continue to use TapRooT® as its corporate root cause analysis tool.

Without the global reach of its former parent organization, it needed to act fast to develop and implement a lean but efficient framework to support and promote the continued use of the TapRooT® tool, during what would be a challenging transition period which included rapid company growth. This strategy included the initial training of staff and managers, ongoing competency and investigation quality assessment and the development of a network of regional facilitator “champions” who would provide local support, quality control and facilitation expertise.

Eric Olson, Senior Advisor – Safety, Health & Environment from Texas, will be sharing Gap Analysis of TapRooT® Implementation and Improvements Implemented.  It is imperative that a global organization operates consistently in the application of all management systems, including root cause analysis. In 2010, Ensco performed a gap analysis of TapRoot® Incident Investigation and Root Cause Analysis and its implementation. This gap analysis was utilized to form a steering committee and work group to identify and set criteria to assist Ensco in attaining a consistent application of TapRoot®. Ensco’s vision states that “we will go beyond what is expected to achieve a safe zero- incident environment” and one of our core values is “no harm to people, property or the environment.” Ensco established formal TapRooT® training requirements and implemented TapRooT® web based enterprise software for use during the TapRooT® process, ensuring proper management of incident investigations to make certain that all incidents are investigated and corrective actions issued in a consistent and reliable method.
