Lessons Learned: Control Lost at Takeoff

An AAIB report details how a pilot lost control upon takeoff ended up underneath his aircraft.

As a pilot was about to taxi for takeoff at Runway 05 at Glassonby Airfield, he was seen to shut down the engine before leaving the cockpit to adjust a camera mounted on the wing.

His plane was a Magic Cyclone (G-KEVA) aircraft, which accelerated and struck a wire fence at Glassonby Airfield, Penrith, Cumbria, August 16, 2023.

As he climbed back into the cockpit from the right side, the pilot reported that he probably knocked the hand throttle forward, which was mounted on the right side of the cockpit. His previous aircraft had the throttle mounted on the left side of the cockpit and this was only the second time he had flown G-KEVA.

The pilot could not remember if he fastened his safety harness. As he restarted the engine, the aircraft immediately accelerated and struck a wire fence and windsock tension cable at the side of the runway, where the pilot was thrown out of the aircraft. The pilot who sustained serious injuries was found trapped under the aircraft.
