Has BP Found the Root Cause?

The Houston Chronicle reports that a six-month internal BP review conducted by the new CEO, Tony Hayward, has found “organizational complexity” as the cause of their operational and safety problems.

Here’s a quote from the article…

CEO Tony Hayward, who assumed the top post in May upon the abrupt resignation of John Browne, led a six-month review of the London-based oil giant’s operational performance, which identified “wide-ranging duplication, overlap and excessive organizational complexity” that caused a competitive shortfall in comparison with its peers, the company said.

What do you think? Was this the cause? What will they do to avoid duplication, overlap, and excessive complexity?
