Five Days of Root Cause Analysis in Amsterdam
Aamer Shamim and I taught the 5-Day TapRooT® Advanced Root Cause Analysis Team Leader Training in Amsterdam, October 19-23, 2026. Over the five days, participants worked through exercises, practiced investigation techniques, and collaborated to turn incident information into actionable improvements.
Putting the Tools to Use
The 5-day course gives participants practice with a structured approach to investigating incidents. Topics include gathering information, organizing an incident’s sequence of events, identifying causal factors, analyzing root causes, and developing effective corrective actions. The training also covers advanced interviewing and evidence collection, deep-diving into the TapRooT® Root Cause Tree, and using TapRooT® Root Cause Analysis for proactive improvement. Participants are given many opportunities to practice what they learned during the course.
Making the Investigation Visible
The SnapCharT® Diagram offers a glimpse into the detailed work behind an investigation. Organizing information visually gives a team a shared reference for discussing what happened, identifying gaps, and deciding what information is missing and still needs to be collected.
For an investigation team leader, helping people work through that information is essential. The exercises create space to practice that process, ask questions, and learn through discussion.
A Classroom Beyond Four Walls
Amsterdam provided an amazing backdrop for the week, and some of the work moved outside. The image above shows participants practicing mock interviews on the terrace.
Bringing the Learning Back to Work
The five days extend beyond completing classroom exercises. It prepares participants to apply a systematic investigation process in their own workplaces and develop corrective actions that address the causes they identify.
