Wow. It’s hard to believe that we’ve already recorded over 75 editions of the TapRooT® Podcast. For International Podcast Day, I wanted to take a moment to look back on why we started this podcast, which editions stand out, and what’s in store for the future.
A Podcast Made for Incident Investigators
The goal of the TapRooT® Podcast is to share root cause analysis best practices with incident investigators.
We want to help companies solve their safety, quality, environmental, and equipment problems, so the podcast provides actionable advice with real-world examples — all packed into bite-sized, 20-to-40 minute episodes.
While plenty of our clientele enjoy the podcast, it’s accessible for everyone, no matter how unfamiliar with the TapRooT® Process.

This episode generated lots of cautionary discussion about the workplace culture seen at OceanGate prior to the Titan’s implosion. It’s a great watch for those who are passionate about innovation without sacrificing safety.
Five-Why Examples
This edition takes a deep dive into popular Five-Why examples, including one developed by the man who popularized the method: Taiichi Ohno. I’d recommend this to both Five-Why users and critics.
Investigative Interviewing
Interviewing for an incident investigation is a challenging but important step in the process. This is a must-watch for those who take evidence collection seriously.
