In most cases, a pipeline failure is a result of decisions or actions made by humans or human performance. TapRooT® RCA is an excellent tool for exposing the root causes of pipeline failures. Only by analyzing the root causes can we truly hope to approach “zero” pipeline incidents and elevate pipeline safety to a new level.

Cary Windler is a self-employed, pipeline safety consultant and Principal with Windler Ventures, LLC> Cary has over 20 years of pipeline integrity management experience and has participated in numerous pipeline failure analyses and root cause analyses. Cary is a licensed professional engineer, System Improvements, Inc. contract TapRooT® RCA instructor, and was formerly a certified aircraft mishap investigator with the USAF.
