IOSH Magazine reported on a series of accidents that could have been prevented.

First, the CEO of a South Korean battery plant received a 15-year prison sentence after 23 workers were killed in a fire (see this LINK). Second, a worker was killed when an unguarded flywheel struck him. The company was fined £1.3 million (CLICK HERE for details). Finally, a company was fined £300,000 after an explosion at its facility in the UK. The video of the explosion is shown below…
