To start, both the instructors and participants introduce themselves. This isn’t merely a formality; our training is inherently collaborative to simulate the modern work world.

After that, the fundamentals are discussed. The instructors cover what skills will be taught over the next five days. This specific course covers both essential tools (SnapCharT®, Safeguards Analysis, the Root Cause Tree®, and the Corrective Action Helper®) and optional additions (Change Analysis, CHAP, and Equifactor®).

As an ice-breaker activity, participants collaboratively build different types of houses using only cards. Once they inevitably crash down, the team leaders must allocate the blame and implement corrective actions.

What’s the fairest way to decide blame? Is the last person to touch the stack at fault? What about the person who designed the structure?

It’s a trick question: blame is useless! The problem shouldn’t be pinpointed on a specific person, but the process they were forced to engage in and the corrective actions implemented without understanding the true root cause.
