With all this information, what can you learn?

The Investigation and Root Cause Analysis

If you are an accident investigator, you need to organize the evidence you have collected. Using the TapRooT® System that starts with your SnapCharT® Diagram.

There was a fire but what were the Safeguards?

Previous flight leaves debris on the runway. Why did the part fall off?

Debris punctured tire. Tire fragments damage the wing and the pressure change causes a fuel tank to leak. The fuel ignites.

Why did this leak result in a fire where previous leaks didn’t catch fire?

The fire melts the ailerons and potions of the left wing.

The jet roles left end crashes on a hotel.

Once your SnapCharT® Diagram is complete, you need to identify the Causal Factors.

Once the Causal Factors are identified, you would analyze them using the Root Cause Tree® Diagram to find the root causes.

This would all be part of the TapRooT® 7-Step Major Investigation Process…
