Success Story From a Refinery

An advanced root cause analysis system was implemented at a large refinery. The program was working well. How could you tell? The first indication was that management began requesting more root cause analyses using the advanced root cause analysis system.
At first, the system was only applied to major safety incidents. However, management started asking for it to be applied to:
- Equipment failures
- Environmental incidents
- Unscheduled process shutdowns
- Precursor incidents
- Turnover project cost and schedule overruns
Soon, they were DEMANDING advanced root cause analysis. Why? Because of the results. They knew they could count on the results from their investigators using the system. They could see better investigations that found root causes that would have been previously overlooked. Plus, management observed more effective corrective actions. But that was just the start.
Next, and perhaps most surprisingly, the Union requested more advanced root cause analysis. They even picketed when a union member was fired without first conducting a root cause analysis. The Union members understood that the advanced root cause analysis wasn’t looking to blame employees. Rather, the system was designed to identify and fix system causes of incidents.
Achieving High-Reliability Requires Advanced Root Cause Analysis
Many consultants don’t understand what is required to achieve high reliability. They have never run a high-reliability organization. They don’t understand what Admiral Rickover meant when he said:
“Success teaches us nothing; ONLY failure teaches.”
That’s why great managers demand advanced root cause analysis. They know that the fastest way to learn is to learn from mistakes, so you never repeat them. After they have a working advanced root cause analysis system, they can start being proactive.
