A Company in Northern Ireland has had the third fatality in since 2009 at the landfills it operates. The first fatality resulted in a £190,000 fine. The second fatality resulted in a £280,000 fine. The most recent fatality resulted in a £60,000 fine.
It seems that fines aren’t enough to make corrective actions happen. After multiple fatalities in a fairly simple operation (burying garbage), it seems like they could learn from their mistakes and stop future fatalities.
