5-Whys and Who Do We Fire?
I received a nice note from a TapRooT® User the other day with an interesting comment. She said her husband called the “5-Whys” technique the:
“5-Whys and Who Do We Fire?”
Why did he say that? He said that’s what he often observed.
Maybe the “ROOT CAUSE” in the figure above should be replaced with the standard corrective action …
“YOU’RE FIRED!“

I learned many years ago from you in the good TapRoot course, that there are other more convince alternatives for getting valuable information, instead of the Whys.
After making in my job more than 250 root failure Analysis I conclude that it´s better not to ask Whys.
